Health & Immunization Forms
Students participating in Intercollegiate Athletics are required to complete both the college’s Physical Exam Form, as well as the Immunization Form.
Students should fill out the front of the physical exam form and have their physician fill out the back. This form MUST be on file in the athletic office before a student is allowed to practice.
Please note, physicals expire in 1 year.
