How can I get a copy of my immunization records that I turned in to
Northern Essex?
Written authorization is required for us to release your records. You can click on the Health Forms link on the left side of the Frequently Asked Questions page and then on the Health Records Request Form link in the middle of the next page (second bullet). Print this form out, sign it and send it to the address on the form with the required $5.00 processing fee.
Can I FAX you my health records ?
Yes. Health records can be faxed to us at 978-556-3160
Can I send you my immunization form even if I am still waiting to have my last Hep.B shot?
Yes. You do not have to wait until all your immunizations are completed before you send in your health forms, especially if you are waiting for a series of shots.
I can’t find my medical records. Where should I look?
If you are having trouble finding your medical records, please try contacting the high school you last attended, the college you last attended, and your primary care physician.
I can’t get an appointment with my doctor within the 30 day limit. What should I do?
If you are having trouble getting an appointment with your doctor within the 30 day time limit, consider contacting a walk-in health clinic in your area.
